Query whether 1 NT is correct or negative X showing 4 hearts is better, over which west could jump to 3H, and east could carry on to game. If 1 NT denies 4 hearts then west should probably just pass or bid 2D.
Its awkward to double with four bad hearts and a stiff diamond. But probably nevertheless double is better than 1NT with just one spade stopper.
